Expanded Adult Residential Care Home

Welcome to Hale O Meleana, an Adult Residential Care Home! We are also registered with the State of Hawaiʻi, Department of Health under the Office of Health Care Assurance for an Expanded Adult Residential Care Home.  The Expanded Adult Residential Care Home has special requirements outlined by the Hawaiʻi Department of Health and families would hire a case worker to oversee the plan of care for the resident.  This plan of care is to ensure coordination of services for the resident who is unable to seek “self preservation” as defined and attested by their physician. Hale O Meleana staff work closely with the case worker to ensure all staff here are trained and following the plan of care.  The Director of Clinical Services along with the Resident Care Coordinators, Medication Aides and Resident Aides give feedback and discuss everyone’s needs with the case worker to look for interventions that are appropriate for your family.  Please feel free to contact us to explore how we can assist in “living every moment!”
